Abstract

ABSTRAK

Latar belakang: Penyakit ginjal kronis atau Chronic Kidney Disease (CKD) adalah kondisi progresif yang mempengaruhi >10% populasi di seluruh dunia atau sekitar lebih 800 juta orang. Hemodialisa merupakan salah satu terapi pengganti ginjal yang paling umum untuk mempertahankan kehidupan pasien gagal ginjal stadium akhir. Data dari Kementerian Kesehatan RI (2018) mengungkapkan bahwa sebesar 98% penderita gagal ginjal menerima perawatan terapi hemodialisa. Pasien hemodialisa sering mengalami kecemasan. Kecemasan dapat diobati secara farmakologi, namun pengobatan scara farmakologi bersifat sementara. Terapi musik secara efektif dapat mengurangi kecemasan pasien hemodialisa, selain tidak memiliki efek samping terapi musik dapat menghemat biaya perawatan.
Skenario kasus: studi kasus dilakukan pada pasien CKD bernama Tn. B yang berjenis kelamin laki-laki dan berusia 51 tahun. Pasien menjalani hemodialisa pertama pada tanggal 4 Juli 2023. Pasien memiliki riwayat penyakit hipertensi. Tn. B mengungkapkan kecemasannya saat menjalani hemodialisa, kemudian mencari terapi nonfarmakologi.
Strategi penelusuran bukti: intervensi terapi musik menghasilakn hasil positif. Terbuksti selama 2 kali melakukan terapi music kecemasan pasien menurun, level fatigue score sebelum intervensi 20, setelah intervensi menjadi 14. Data objektif didapatkan pasien tampak lebih rileks, lebih tenang, tampak cemas menurun, tampak tenaga meningkat.
Pembahasan: asuhan keperawatan pada Tn. B meliputi 2 sesi hemodialisa di Rumah Sakit Tk. III 04.06.04 Slamet Riyadi Surakarta. Masalah terkait kecemasan membaik dengan berkurangnya kekhawatiran, kegelisahan, dan ketegangan dikonfirmasi oleh data objektif dan subjektif.
Kesimpulan: masalah kecemasan keperawatan didapatkan hasil bahwa masalah kecemasan atau kelelahan teratasi sebagian dengan data subjektif pasien mengatakan lelah menurun, lesu menurun, tenang sedikit meningkat. Data objektif didapatkan yaitu pasien tampak leb rileks, lebih tenang, tampak lesu menurun, tampak tenaga meningkat, nilai kecemasan 14.

ABSTRACT

Background: Chronic kidney disease (CKD) is a progressive condition that affects >10% of the population worldwide or around 800 million people. Hemodialysis is one of the most common kidney replacement therapies to maintain the life of patients with end-stage renal failure. Data from the Indonesian Ministry of Health (2018) revealed that 98% of kidney failure sufferers received hemodialysis therapy treatment. Hemodialysis patients often experience anxiety. Anxiety can be treated pharmacologically, but pharmacological treatment is temporary. Music therapy can effectively reduce anxiety in hemodialysis patients, apart from having no side effects, music therapy can save treatment costs.
Case scenario: a case study was conducted on a CKD patient named Mr. B is male and 51 years old. The patient underwent first hemodialysis on July 4 2023. The patient has a history of hypertension. Mr. B expressed his anxiety when undergoing hemodialysis, then sought non-pharmacological therapy.
Evidence-finding strategy: music therapy interventions produce positive results. It was proven that during the two times of music therapy the patient's anxiety decreased, the fatigue score level before the intervention was 20, after the intervention it became 14. Objective data was obtained that the patient looked more relaxed, calmer, his anxiety appeared to decrease, his energy appeared to increase.
Discussion: nursing care for Mr. B includes 2 hemodialysis sessions at Tk Hospital. III 04.06.04 Slamet Riyadi Surakarta. Anxiety-related problems improved with reduced worry, restlessness, and tension confirmed by objective and subjective data.
Conclusion: the problem of nursing anxiety showed that the problem of anxiety or fatigue was partially resolved with the patient's subjective data saying that fatigue decreased, lethargy decreased, calmness increased slightly. Objective data obtained was that the patient appeared more relaxed, calmer, appeared to be less lethargic, appeared to have increased energy, and an anxiety score of 14.
